WAIVER
In consideration of the fee paid, 2ND2NUNN Sports, Inc. agrees to provide to participant youth fitness training at the above activity (“Class”). In exchange, participant understands that there is a risk of personal injury in the course of instruction and, with this knowledge, agrees to assume the risk of any injury and damages to participant during the class.
Specifically, participant agrees to hold harmless the School and all other individuals, organizations, sponsors, promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees and other participants connected with the class from all losses, damages, injuries, causes of actions, claims, or complaints in the event that the participant is damaged or injured in any way during the participation, instruction and/or performance of any exercise or during any activity associated with the class location or during transit to or from the class.
Participant further agrees to strictly obey instructors and observe safety rules.
Because of the physical demands of this program, participant understands that he/she must be in good physical condition to participate in the class. Participant understands that in case of injury, the only medical treatment 2ND2NUNN Sports, Inc. will provide is first aid.
Participant agrees that any pictures, audio, or visual recordings taken of him/her in connection with the class can be used for publication, promotion, articles, shows and advertisement without additional consent and without compensation at this time or any other time.
I have read and understand this release and agreement and agree to its provisions. I am not under their influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity. I am over 18 years of age. (If not over 18 years of age, parent or guardian must sign.)
MEMBERSHIP CANCELLATION POLICY
(Membership-Based Instructional Programs)
GENERAL PROGRAMS
(All Programs Unless Otherwise Noted)
2ND2NUNN Sports, Inc. operates membership-based, instructional programs only. All programs are delivered through scheduled, instructor-led instruction and are not open to the general public.
Participation, enrollment, or payment constitutes acceptance of the applicable program terms, whether or not a separate agreement is signed.
Notice Period

Programs may include an initial commitment period, as disclosed at enrollment.

After any required commitment period, members must provide 30 days written notice to cancel.

If notice is received within the final 30 days of a billing cycle, one final payment will be due.

Cancellation becomes effective after the notice period is completed, not on the request date.
How to Cancel
All cancellation requests must be submitted in writing.
Verbal cancellations are not accepted. Cancellation is not valid until written notice is received and acknowledged.
BASEBALL PROGRAM – SPECIAL NOTICE
The Baseball Program operates under the 2ND2NUNN Annual Baseball Player/Parent Contract and is subject to separate terms.

Baseball enrollment constitutes a 12-month, non-cancelable, non-refundable financial obligation.

Monthly payments, if selected, are installment payments toward a single annual obligation, not a recurring membership.

Withdrawal from participation does not stop billing or reduce financial responsibility.

Agreement to Baseball Program terms is established upon any of the following:

Signing the contract

Payment of any portion of the commitment fee

Acceptance of a roster spot

Participation in team activities or use of services

Failure to sign the contract does not void the financial obligation once participation or payment occurs.

To prevent renewal for the next season, 45 days written notice prior to contract expiration is required.
